This closed intervention, performed without the need for open-heart surgery and without creating any incision in the chest, permanently resolves the congenital problem of pulmonary valve stenosis (pulmonary valvular stenosis). The interventional method mechanically opens the narrowed valve, immediately restores blood flow to normal, and prevents the heart from becoming overworked. Offering a low risk rate and the advantage of rapid recovery, this technique is currently the most modern standard treatment that safely protects heart health.

What Kind of Problem Is the Narrowing in the Heart (Pulmonary Stenosis) Before Pulmonary Balloon Treatment?

The functioning of the heart resembles a perfectly designed plumbing and pumping system. Blood that has circulated through the body and become low in oxygen collects on the right side of the heart, specifically in the chamber called the right ventricle. The sole function of the right ventricle is to pump this oxygen-poor blood forcefully toward the lungs so that it can be oxygenated. The main gateway through which blood passes during this pumping process is the pulmonary valve, located directly at the entrance of the pulmonary artery. In a healthy heart, this valve opens fully as blood is pushed forward and then closes tightly to prevent blood from flowing backward. However, in the condition known as pulmonary stenosis, the valve leaflets are fused together, thicker than normal, or underdeveloped because of a congenital structural abnormality.

This structural defect prevents the valve from opening and creates a serious obstruction as blood passes into the lungs. To push the same amount of blood through a very narrow opening, the right ventricle must work much harder than normal and generate considerably higher pressure. Like any muscle in the human body, the heart muscle begins to undergo structural changes when continuously exposed to excessive workload. If this condition is not identified and treated in time, it marks the first stage of a process that may completely impair the functioning of the right side of the heart.

What Risks May Develop If This Obstruction in the Heart Is Left Untreated and Pulmonary Balloon Treatment Is Not Performed?

As the heart muscle struggles to force blood through a narrowed valve, it initially begins to thicken as a defensive mechanism. Progressive thickening of the muscle causes the internal volume of the chamber to decrease and its flexibility to be lost. After years of excessive strain, the right ventricle may eventually fail and begin to enlarge like a balloon. This enlarged chamber can also disrupt the structure of the other heart valves, creating conditions for blood to leak backward into the right atrium.

If the condition is not treated in time, the heart’s electrical system may also be affected by this enlargement, and dangerous rhythm disorders may begin to develop. Blood passing at high speed and with turbulence through a narrow area can damage the surface of the valve and create conditions that increase susceptibility to infections of the heart lining. In the final stage, heart failure may become unavoidable during the progression from childhood into adulthood.

Common clinical symptoms include:

Bluish discoloration

Easy fatigue

Shortness of breath

Chest pain

Palpitations

Feeling faint

Growth retardation

Loss of appetite

Excessive sweating

How Is the Severity of the Disease Classified When Deciding on Pulmonary Balloon Treatment?

To determine how severe the narrowing in a child’s heart is, a completely painless ultrasound device called echocardiography, which works using sound waves, is used. This device allows the speed created as blood passes through the narrowed valve and the pressure difference produced by this speed to be measured with millimetric precision. This pressure difference, called the “gradient” in medical terminology, is the most fundamental measurement used to determine the severity of the disease. The higher the pressure difference, the greater the obstruction in front of the right ventricle.

According to these pressure values, the child’s condition is classified and a treatment plan is established. In mild stenosis, the pressure difference is below 40 mmHg, and these children can continue their lives completely normally without requiring any intervention. In moderate stenosis, the pressure is between 40 and 65 mmHg; if the child has developed symptoms or the pressure exceeds the 50 mmHg threshold, the procedure is planned. In severe stenosis, the pressure is above 65 mmHg, and even if the child has no symptoms, intervention should be performed as soon as possible to prevent permanent heart damage.

What Is Pulmonary Balloon Treatment, Which Is Frequently Mentioned in Pediatric Cardiology?

In previous years, solving any problem in the heart required opening the chest, stopping the heart, and keeping the child in intensive care for days. However, closed methods, one of the greatest advances offered by modern medicine, have completely changed this process. This treatment is based on reaching the heart through a vein in the groin using a very thin and flexible tube called a catheter in specially equipped technological rooms known as angiography laboratories.

At the tip of this thin tube is a special balloon that remains completely deflated until the procedure begins. The tube is advanced to the right side of the heart and passed through the exact center of the narrowed valve. Once the target point is reached, the balloon is inflated under high pressure using a special liquid delivered from outside the body. The inflated balloon gently but firmly separates the valve leaflets that have been fused together since birth. The valve opening is widened within seconds, and the severe burden on the right ventricle is immediately relieved. This method, which leaves no incision scar on the chest, is the universally accepted first-line treatment for relieving the stenosis.

Why Is Pulmonary Balloon Treatment Much More Urgent and Vital in Newborn Babies?

The lungs of a baby in the womb are not yet filled with air, and the baby receives oxygen through the mother’s umbilical cord. Therefore, even if the pulmonary valve in the baby’s heart is completely blocked, this condition does not affect the baby’s development while still in the womb. However, the moment the baby is born and takes the first breath, the entire circulatory system changes within a second. Blood must begin flowing to the lungs. If a newborn baby’s pulmonary valve is severely narrowed, blood cannot reach the lungs and the baby immediately begins to turn blue.

In newborns with this condition, there is only one small residual vessel that can keep them alive. Immediately after birth, a special medication is urgently administered to keep this temporary vessel, which would normally close, open. As soon as the baby’s condition is stabilized for several hours with this medication, a race against time begins. The baby must urgently be taken to the laboratory using millimetric instruments suitable for the tiny heart, and the valve must be opened.

Conditions requiring urgent intervention in newborns include:

Severe bluish discoloration

Circulatory collapse/Shock

Respiratory distress

Rapid breathing

Low blood oxygen level

Inability to feed

Persistent drowsiness

How Is the Pulmonary Balloon Treatment Process Affected When the Valve Structure Is Abnormal (Dysplastic)?

Not every case of pulmonary stenosis is identical. In approximately ten percent of children, the valve is not only fused but has also become a rigid, excessively thick, and irregular mass of tissue because of a congenital structural abnormality. This condition, which is generally associated with certain genetic syndromes, is called a dysplastic valve. The ring in which the valve sits may also be significantly narrower than normal.

When this type of anatomy is present, balloon treatment is much more difficult than in standard cases. The inflated balloon may be insufficient to stretch and open this thickened, rigid tissue that has completely lost its flexibility. Even if the narrowing is partially widened with the balloon, the likelihood of this rigid valve becoming narrow again within months or several years is quite high. In these very specific and resistant cases, close monitoring of the child and, when necessary, surgical removal of the thickened tissue may offer a more permanent solution.

What Preparations Are Made in the Hospital Before Pulmonary Balloon Treatment?

The thought of an intervention being performed on a child’s heart is naturally concerning for families. However, this process is carried out in a highly planned, step-by-step manner within a broad safety framework. The procedure is generally planned as a day-case admission or with only one night in the hospital. The child is evaluated by specialist anesthesia teams to ensure that no pain, discomfort, or fear is felt during the procedure. The intervention is performed under full general anesthesia while the child is in a deep sleep.

On the morning of the procedure, the child is asked to remain without food or fluids for a specific period. Detailed examinations are performed to ensure that all bodily values are completely suitable for the intervention. The current condition of the child’s heart, the exact structure of the valve, and the size of the right ventricle are carefully measured one final time, and the procedural strategy is confirmed.

Routine tests requested before the procedure include:

Complete blood count

Coagulation tests

Kidney function tests

Liver enzymes

Electrocardiography

Echocardiography

Chest X-ray

Blood group determination

Technically, How Is Pulmonary Balloon Treatment Performed Step by Step?

After the child is taken into the angiography laboratory and falls asleep, the groin, which is the procedure site, is completely sterilized using special solutions. To reach the heart, the wider and safer vein in the groin is used rather than the artery. A plastic sheath through which all catheters can pass comfortably is placed inside this vessel. This helps prevent damage to the vein.

A very thin hollow tube is then advanced through this vein upward into the right atrium and from there into the right ventricle. With advanced imaging devices that operate using X-rays, the movement of this tube is monitored second by second on large screens. When the tube reaches the narrowed valve, the high pressure inside the heart is measured using a special device. A small amount of harmless contrast dye is then injected into the vessel to obtain a moving image of the valve. This image clearly reveals the shape and diameter of the valve.

Why Is the Size of the Balloon Used During Pulmonary Balloon Treatment So Important?

There is one mathematical formula that determines the success or failure of this delicate intervention: the size of the selected balloon. During this step, which must be decided within seconds on the angiography table, physicians measure the diameter of the ring where the valve is located with millimetric precision. The balloon used must always be slightly larger than the valve’s own diameter. The ideal ratio is for the balloon diameter to be twenty to forty percent larger than the valve diameter.

If a balloon that is smaller than necessary is selected out of concern about opening the stenosis too aggressively, the adhesions in the valve cannot be fully separated and the narrowing in the child’s heart continues. However, if an excessively large balloon is selected in an attempt to obtain faster results, there is a risk of tearing the valve, damaging the heart’s outflow tract, and creating severe leakage that may place considerable strain on the heart in the future. The length of the balloon must also be selected very carefully, as an excessively long balloon may come into contact with delicate structures in the other chambers of the heart.

For this reason, the physician may sometimes prefer a two-stage procedure instead of widening the valve completely in a single session. A balloon slightly smaller than the normally required size may be used to reduce the stenosis without creating valve insufficiency, with the remaining narrowing treated later through a second angiography procedure.

What Happens to the Valve When the Balloon Is Inflated During Pulmonary Balloon Treatment?

Once the balloon with the most appropriate mathematical dimensions has been selected, it is advanced over a thin guidewire and positioned precisely in the center of the valve. The balloon is inflated not with air but with a special liquid mixture using an external pressure pump. The image on the screen is extremely clear at that moment: As the balloon begins to inflate, the narrowed and rigid valve squeezes the center of the balloon, giving it an hourglass appearance.

As the pressure is increased second by second, the fused valve leaflets can no longer resist. On the screen, the constricted section creating the hourglass appearance is seen to suddenly open and become a completely smooth cylinder. This is the moment when the stenosis disappears, the adhesions separate, and the heart finally experiences relief. To avoid interrupting blood flow, the inflated balloon is rapidly deflated within seconds, and the procedure may be repeated several times if necessary to ensure that the valve has opened completely.

What Immediate Reactions May Develop in the Heart After Successful Pulmonary Balloon Treatment?

For years, the right ventricle has built an enormous muscle mass to push blood through the narrow opening in front of it. When that opening is suddenly widened completely with the balloon procedure, the thickened muscle structure may not immediately adapt to the new, less demanding situation. Even though the obstruction at the valve has been removed, the excessively developed muscle tissue immediately below the valve may contract reactively as blood passes through, creating a temporary and dynamic narrowing beneath the valve.

This is a completely normal and expected physiological response. To reduce this excessive contraction and stress within the heart and allow the thickened muscle tissue to relax, the child is generally started on an oral relaxing medication immediately after the procedure. With the support of this medication, the excess muscle tissue gradually regresses over several months, temporary contractions disappear, and the pressure inside the heart returns to levels seen in a healthy person.

What Are the Main Advantages of Pulmonary Balloon Treatment Compared with Open-Heart Surgery?

Historically, before balloon technologies were developed, open-heart surgery was the only option for these children. Today, it is almost impossible to compare the two methods. Open surgery requires cutting the breastbone, connecting the child to a heart-lung machine, completely stopping the heart, and leaving a lifelong scar on the chest. It also involves a painful recovery period lasting several days in intensive care.

None of these occur with the balloon method. The chest is not touched, and only a needle-sized opening is created in the groin. The child remains in a deep sleep throughout the procedure, and the heart is not stopped. Blood transfusion is not required, and the risk of infection is almost zero. One of the greatest advantages is that if the valve becomes narrow again years later, a second balloon procedure can be performed with the same ease and safety, whereas reopening the chest for a second time always carries much greater risks.

The main advantages include:

No incision scar

Short hospital stay

Rapid recovery process

Low risk of infection

No need for blood transfusion

No psychological trauma

No need for intensive care

The procedure can be easily repeated

How Long Does Hospitalization and the Return to Normal Life Take After Pulmonary Balloon Treatment?

When the procedure is completed, the thin tube in the groin is removed and gentle pressure is applied to the area for a while to stop bleeding. As the child gradually wakes up in the recovery room, only very mild soreness may be felt in the groin. On the day of the procedure, the child is generally asked to lie flat in bed for several hours to prevent the groin from bending. However, by the evening, the child can usually stand up, walk comfortably, and begin eating.

The child generally remains in the hospital for only one night for observation. The next morning, a final heart ultrasound is performed before discharge to confirm that everything is progressing normally, and the family can return home with reassurance. The small entry point in the groin heals completely within a few days. A short period of rest at home for several days is generally sufficient before the child returns to school, play, and social life.

Points to consider at home after discharge include:

Drinking plenty of fluids

Keeping the groin dry

Keeping the groin clean

Briefly avoiding strenuous exercise

Using prescribed medications

Monitoring for fever

Paying attention to nutrition

What Is Monitored During Long-Term Follow-Up After Pulmonary Balloon Treatment?

Although mechanically opening the valve resolves most of the problem, complete cellular recovery of the fatigued heart muscle takes time. For this reason, children are called for regular cardiac check-ups at designated intervals. During these examinations, not only the condition of the valve but also the contraction and relaxation functions of the right ventricle are evaluated in detail.

The right ventricle, which has struggled against high pressure for years, may retain very slight differences in its ability to relax even after the pressure is relieved. Special ultrasound measurements are used to assess values showing the strength and duration of heart muscle contraction. Detailed measurements may reveal millimetric differences in certain heart movements compared with healthy children. However, the most important point to understand is that these subtle differences detected by medical devices do not negatively affect the child’s daily energy, ability to run, or growth.

Is the Development of Valve Leakage (Insufficiency) After Pulmonary Balloon Treatment Dangerous?

As the balloon is inflated and the valve is widened, the fused valve leaflets necessarily separate through small tears. The most natural and unavoidable consequence of this process is that the valve may not close completely and some blood may leak backward into the right ventricle. This backward leakage, known as pulmonary insufficiency, is one of the issues that most concerns families after the procedure, but it is actually an expected outcome.

However, the human heart, particularly the right side of the heart, has an excellent ability to tolerate this type of backward leakage. Long-term scientific follow-up has clearly shown that the overwhelming majority of these leaks are very mild. They do not strain the heart, reduce the patient’s exercise capacity, or worsen over the years. Physicians carefully monitor the severity of the leakage during follow-up, but in most cases it does not prevent the child from living a completely normal life.

In Which Situations Is Open-Heart Surgery Preferred Directly Instead of Pulmonary Balloon Treatment?

Despite its remarkable results, there are rare situations in which closed methods are insufficient and surgery is required. Particularly in children with the previously mentioned abnormally thick and fleshy dysplastic valve structure, balloon treatment may not create the desired opening. If the pressure difference does not decrease despite repeated balloon inflation, further force is avoided and surgery is planned.

Surgery is also necessary if the obstruction is caused not only by the valve but also by a severe congenital thick muscle bundle immediately below the valve, or if the pulmonary artery itself is narrowed above the valve, because the balloon cannot remove this muscle tissue. During surgery, the heart is stopped, the obstructing area is removed, and the valve region is reconstructed with special synthetic patches when necessary. Apart from these very specific anatomical conditions, closed methods are always the first choice.

Frequently Asked Questions

What is pulmonary balloon treatment and for what purpose is it performed?

Pulmonary balloon treatment is an interventional procedure performed to relieve pulmonary valve stenosis.
